Forms, applications, guidelines and other tools are available for download on this site in PDF Format and MS-Word.

For PDF Format: you must have Acrobat Reader installed in order to view these documents. You can download Acrobat for free at Adobe.com. This version of the application allows you to print it and then you must use a typewriter to fill it out.

For MS Word Format: This version allows you to fill out the application on your computer and then print it out. If you have any problems or questions, please email the DEC staff at artsgrants@fltg.net.

Please remember, any application received by this office from an organization which has NOT attended a DEC Seminar will be considered INELIGIBLE for review and will NOT be considered for funding. See the DEC Seminar schedule for the most convenient date, time and location for your needs.
